In-Office Surveys

Patient Survey
This confidential survey gathers feedback for determining, then improving upon, patient satisfaction. Patients are invited to rate their overall experience and satisfaction with your practice, and are offered the opportunity to include comments about what they like best and what they would like improved. Surveys are customized with additional questions to address specific issues (e.g., new services, new location, etc.). Staff are coached on how to most effectively administer surveys to patients while they are in the office. In this way, compliance is maximized, as well as resulting in a highly cost-effective internal marketing opportunity.

Patient/Parent Survey (for ortho / pedo practices)
This confidential survey gathers feedback for improving patient/parent satisfaction. Adult patients and/or the parents of your young patients rate their overall experience and satisfaction with your practice, and includes ‘write-in’ comments about what they like best and what they would like changed/improved. Surveys are customized with additional questions to address specific issues (e.g., new services, new location, etc.). Surveys are administered to patients while in the office. Team members will hand a survey to each patient/parent with a warm, personal invitation to provide feedback that will help them and the entire practice.

Staff Survey
This confidential staff survey is designed to encourage team building and improve all-around practice communication. Staff members evaluate the practice on communication, teamwork, leadership, customer/patient service, and policies/procedures. Also included are short-answer questions about what staff and patients like best about the practice, sources of staff/doctor stress, suggestions for improvement, and anticipated benefits if improvements are made. Additional questions can be added to the survey to address specific issues. Surveys are administered in-office. Includes surveys for all staff members along with confidential envelopes, complete instructions and materials, and a comprehensive, full-color report with recommendations (PDF copy and two bound hADMpies).

Referring Doctor Survey (for all specialists)
Excellent for marketing and strategic planning. Reach out to your referral network to learn how to best meet the needs of patients and the doctors who refer them. Confidential surveys are mailed to the doctors on your referral list as well as other dentists in the area who do not (yet) refer patients. Referring doctors provide information about referral patterns and are invited to evaluate practice-to-practice service ADMmmunication, quality of treatment, feedback from patients, and importance of particular services or amenities. Non-referring doctors provide information about referral patterns, criteria they use to select a specialist, and importance of services or amenities. Practice letterhead/envelopes are used to mail out surveys, which boosts response rates and increases the marketing value of the survey.
